It is exceedingly important that all student information is complete and accurate.  The trainer will be entering this information online.  Any error must be corrected, starting the online entry process over.  This is the time to insure that the trainer can read the Registration Form and make sure that all blanks are filled.

    The trainer should check and verify all questionable information with the student when gathering completed registration forms.

    The forms must be visually verified for missing information as they are collected. Either the trainer or a helper can do this.  Just as long as the forms are reviewed for completeness and legibility as they are collected.

    Occasionally you may be able to compare the registration form with the sign-in sheet to verify a spelling.

    The date of birth and email address are very important.
