The Chain of Custody Form is usually provided by the laboratory that the student chooses to analyze paint chip samples.

This is a form from the lab that Seagull uses.  It is only meant as an example.  It is not the case that students must use this particular form in the field.  It should only be used during training.

The Chain of Custody Form is used as proof that the sample was not tampered with from the time the sample was taked until it reaches the laboratory.

Each initial and refresher student should be provided with a Chain of Custody Form.  How to use it is part of the Seagull hands-on activities.
