The “Combo Form”, which is the nickname for the “Registration, Sign-in, Evaluation, & Examination Form”, is used for all regularly scheduled Seagull RRP training.  It replaces all other hard-copy forms. 

The Combo Form is divided into three sections:

1. 1. The student fills in this information and signs it at the beginning of the course.
   

2. 2.This section is filled in by the hands-on instructor which may be at a remote location.
   

3. 3.This section is for whomever checks the student’s ID and is aware that that student jumped through all of the hoops required.  It must be signed.
   

Once the student has filled in his part, the instructor or facilitator must check for legibility, this is our only hard-copy back up.  This form is our testimony that the course was accomplished in accordance with regulations.  It is very important!
